Thank you for purchasing a custom map! I'm sure you are excited to get the design process started and see your vision brought to life! I have created this form to gather all the information I will need to create your custom map, from the first sketches to the final design. 

At the end of the form, you will also see a design contract that lays out in detail how the design process will work. That way, we can be on the same page throughout all stages of the design process. Hopefully the contract will answer your questions about sketches, requesting changes, proof approvals, and how long everything will take, but if you're confused about anything, please ask for clarification.

Design Contract

I will send you the first version of your map 2-3 business days after you complete this questionnaire. You may request changes to the map design via email, and are allowed a total of three rounds of revisions. Each email with requested changes counts as a "round" of revisions, so you are allowed three emails with change requests. You can request as many changes as you'd like in each round, but all the requests must be sent in a single email. If you forget something and send another email to add it, that counts as another round of revisions. If you need more than three rounds to get the map right, you will be charged an additional fee. *
You do not have to use all three revision rounds, and may approve the map as finalized at any point in the design process. *
The $250 purchase price is only for a digital file of the map sent to you via email, not a physical printed item. If you would like printed maps, an additional fee will be incurred; it is charged in a separate transaction from the $250 design fee. *
You may not alter the map design in anyway, or re-use or re-incorporate a part of it into another project without my consent.
The buyer holds sole responsibility for any mistakes or misprints after receiving the final files. *
The map design is for personal use only, and may not be sold or used for commercial purposes. *
